Please click here to download the file We’re here to provide expert care, comfort, and all-encompassing support to children with life-threatening conditions and their families, helping them enjoy the best possible quality of life. Our Community Nursing Team plays a vital role in linking hospital, home, and hospice care offering tailored support where and when you need it most. They work alongside you every step of your journey, building strong relationships that help make care feel consistent and connected. Things our nurses can offer include: Emotional and practical support for you and your whole family Clear, easy-to-understand guidance on your child’s care Being there with you during hospital or medical appointments Helping you think through care plans and choices about your child’s care Staying in touch with hospital teams so everyone works together Helping manage symptoms to keep your child as comfortable as possible Gentle, thoughtful support around end-of-life care, if needed The impact on your child Our nurses focus on more than just clinical care. They work closely with you and your family to understand your child’s condition, wishes and needs – and help plan care that not only makes them feel comfortable at every stage, but enables them to be children first and foremost - e.g. to have fun, play with siblings, go to school, make memories. They’ll sit with you and discuss all of your options, so your nurse can ensure your child has the right care in the right place to best support them. They may also help children better understand their condition, if appropriate. The impact on you and your family Community Nurses build strong, trusting relationships with you and your family whether that’s at home, in hospital, or in one of our hospices – so that you can enjoy moments together as a family, not carers. They’re there to remind you that you have a network of support around you. They can help you address any worries or concerns you may have about hospice care, and signpost you to wider support services, both at the hospice and in the community. Where and when support takes place Our Community Nurses work across hospitals, homes, and our two hospices. They are based locally, and work closely with the local children’s community nursing teams. This means you’ll usually work with the same nurse throughout your child’s journey – offering you consistency and familiarity. While our nurses are based locally, they can also work with hospitals outside of our catchment area (such as Birmingham, Alder Hey etc.) ensuring you and your child receive support no matter where you are. How to access community nursing support Children must meet our eligibility criteria and be approved through our referral panel before full hospice services can be accessed. Referrals can be made by hospital consultants, GPs and other healthcare professionals, or yourself. However, our Community Nurses can support you and your family through this process, even before a child is officially accepted.
