Our Criteria For care: At Hope House and Ty Gobaith, referrals are considered by a team that includes the hospice medical officer and senior nurses from both hospices. Their role is to carefully consider and assessed each referral in line with our criteria, policies, and the individual care needs of the child or young person. Part of this process involves looking at information from the child's medical professionals to understand their condition and the support they may need now and in the future. We care for babies, children and young people from Shropshire, Cheshire and across Wales who are living with a wide range of life‑threatening conditions, including: Category One Life-threatening conditions for which curative treatment may be feasible but can fail, where access to palliative care services may be necessary during treatment and/or when treatment fails. Children in long-term remission or following successful curative treatment are not included. Category Two Conditions where premature death is inevitable, where there may be long periods of intensive treatment aimed at prolonging life and allowing participation in normal activities. Category Three Progressive conditions without curative treatment options, where treatment is exclusively palliative and may commonly extend over many years. Category Four Irreversible but non-progressive conditions causing severe disability leading to susceptibility to health complications and likelihood of premature death. The family must live in Shropshire, Cheshire, Mid or North Wales, but do not need to have had any previous contact with Hope House Children's Hospices. For more information please get in touch. It is also available to families whose child was previously registered with Hope House Children’s Hospices (aged between 24 weeks gestation and their 19th birthday) within five years of discharge.
