Teamwork Makes The Dream Work For Nia A nurse Tŷ Gobaith has won a special award for her dedicated and passionate work in the community. Nia Jones, Community Clinical Nurse Specialist at Tŷ Gobaith in Conwy was named as winner of the Partnership Award at the recent Betsi Cadwaladr University Health Board Achievement Awards. At the glitzy evening held at Venue Cymru on Friday September 26th, Nia was honoured alongside the Trust’s Central Integrated Health Community (IHC) Children’s Team who cover Conwy and Denbighshire. There were 475 members of Health Board staff, volunteers and sponsors in attendance and 30 finalists in a host of categories. The Partnership Award celebrates individuals or teams who bring people together to make things happen, while demonstrating excellent practice in working towards a shared goal. Nia (second right) with Partnership team colleagues at the BCUHB awards night As part of her role, Nia works closely with the Central IHC Community Children’s Team to support children with life-limiting conditions across North Wales. Together, they take a joined-up approach to care – making sure to always place the needs of children and their families at the centre. By working as one team, they’re not only enhancing care for children with complex needs but also reducing the need for hospital admissions by delivering specialist support in the community. Since Nia’s role became part of the team, families have also gained better access to counselling, social work, and emotional support – ensuring holistic care when it’s needed most. Nia shared: “I’m so proud of the work the whole team do for children and families across North Wales. “I’m thrilled to accept this award for everyone involved. “I look forward to helping drive forward care for young people with life-threatening conditions. “It’s a privilege to do the work we do. Thank you.” The success of this collaborative approach has been so significant that there are now plans to expand it across the west of the health board, meaning even more families will benefit from this model of care.
